Information File

By law all clients are required to provide their personal details and information regarding their health for any treatment service they receive. Your details are not shared and kept for your therapist reference only at Relaxation Station. **Under 16 are considered a minor and must have parental consent to receive a treatment. Signing this form allows consents for your under 16 to receive suitable treatments, along with workshops/ events or treatments they may not be accompanied by yourself.

Patch Testing Requirements

Patch testing for some services are required. This is to minimise your risk to sensitivity/ allergies/ reactions to certain products or procedures. Please fill in this form if you are required to receive or have completed a patch testing with us.

Patch tests are provided a minimum of 24-48 hours before you receive the new treatment service. If you show any signs of a reaction contact your doctor immediately.

Patch testing requires you to inform us of any reactions occurred. A reaction can take anything from 1-24 hours to occur. Please provide Relaxation Station with images of any potential reaction. A re test maybe required. Contact your doctor if you do show any signs of a reaction immediately. Any new medication or health concerns may require you to re test before any of these services even if you have previously tested negative.

Reflexology Consultation

Post reflexology you may experience any of these symptoms for up to 24 hours. This is a natural sign of your body self healing and processing. Be kind and relax and listen to your body's needs. Hydrate and rest:

Lethargy, mood swings, increased or decreased energy, hot flushes, feeling cold, sleepy, Increased bowl movement, thirst, body aches, headaches, hunger.

A course of reflexology works best to re algin any concerns. Any post symptoms will minimise if occurred previously. Discounts are provided on courses t&c's apply.

I understand the benefits and contractions that can occur with reflexology. I will inform Relaxation Station if any of these details change, verbally or via re submitting a new consent.

Bespoke Product Making

This form collects details for your bespoke product making needs. Please fill in as much information detail as possible. I will contact you to discuss your needs and plan your products. Turn around time is 1-3 weeks ( subject to ingredients supply). All products are to be pre paid for. The charge for this bespoke service includes, consultation, formulation and regulatory standards. Fee £60 for this service. Each item is charged additionally as stated/quoted. Please submit your fee along with this form where possible. Alternatively an invoice will be sent with confirmation of your order. Bespoke fee and products must be paid for in full before formulation. Postage additional if required.
